Theoretical orientation
My therapeutic orientation is eclectic/humanist. That means that the focus of psychotherapy is exclusively on you and your goals; I also incorporate cognitive/behavioral and insight-oriented methods into my work. Above all, you should expect that my orientation as a therapist is interactive - both to support and challenge you in attaining the goals you seek in therapy.
Confidentiality
Dr. Haldeman's psychotherapy practice conforms to the laws regarding confidentiality for psychological services as mandated by Washington State law. All information relative to treatment is strictly confidential, with specific exceptions (suicidal or homicidal intent, grave incapacitation, or reported abuse of a child or elder person). A signed release of information is required before any information about psychological treatement can be released to another health care provider, or other interested party.
Insurance, Fees and Cancellation Policy
I bill most insurance companies directly for psychotherapy services. This saves you the inconvenience of paying me up front, filing an insurance claim, and awaiting reimbursement. I participate in a variety of insurance panels, including Premera Blue Cross, Regence Blue Shield, Aetna, United Behavioral Health, United Healthcare, Uniform Medical, and First Choice Health. However, you should check with your insurance company regarding what your benefits are for outpatient psychotherapy - deductible, number of sessions per year, and whether or not the benefit is managed by the insurance company. If you have a managed benefit for outpatient mental health, you must obtain an authorization prior to seeing me.
As of May 1, 2005, my fee is $130 for a 50 minute psychotherapy session. Appointments that are cancelled with less than 48 hours notice are charged at the full fee, excepting serious emergencies.
FAA Evaluations
In addition to psychotherapy, my practice includes conducting psychological evaluations of pilots and air traffic controllers for the Federal Aviation Administration. Pilots needing a psychological evaluation in partial fulfillment of the requirements for reinstatement of their medical certificates should contact me directly for an interview and testing appointment. Generally, a four to five hour appointment for interview and testing is required, as well as several hours of data analysis and report preparation. Relevant treatment records should be forwarded directly to me at least one week in advance of the appointment. As insurance coverage for this service is quite variable, fees for the evaluation must be paid at the time of service.
